NHS Forth Valley Survey Terms and Conditions

Terms and Conditions
  1. Closing date for Entries is 12 noon on May 26th 2010
  2. To qualify to enter the competition you must be resident in the United Kingdom and be 18 years old or over. You must not be an employee, an agent or a director of Scottish Health Innovations Ltd (SHIL), its associated companies or subsidiaries or any member of any such person's family. SHIL reserves the right to verify the legibility of applications.
  3. No entrant will be included after the deadline. SHIL, however, reserves the right to change the deadline.
  4. All entrants are deemed to have accepted the rules for the competition by submitting a questionnaire. The rules include the terms and conditions and information in the questionnaire, accompanying email and leaflets.
  5. The winning entrant will be picked at random by a member of the SHIL team.
  6. By entering the competition, you hereby warrant that all the information submitted by you in the survey to be true.
  7. All entrants must be employees of the Forth Valley Health Board.
  8. SHIL and the National Waiting Times Centre reserve the right to publish the winners name in promotional material for following events and publicity.
  9. Three winning entrants will receive a cash prize of either £50, £20 or £10..
  10. SHIL reserves the right to amend, alter or cancel the competition without prior notice.
  11. The judges’ decision is final and no discussion will be entered into on the decision of the Judges.
  12. The name of the winner will be available from SHIL after the closing date, on written application enclosing a Stamped Addressed Envelope.
  13. All surveys received will be treated as confidential by SHIL.
  14. No responsibility can be accepted for entries lost or delayed in transmission whether by email or otherwise. SHIL has no responsibility for damage or loss resulting from misdirected or incomplete entries arising from computable functions, virus, bugs or other causes outside its control.
  15. SHIL reserves the right to disqualify any applicant if it has reasonable grounds to believe the applicant has breached any of these terms and conditions.
  16. The winners may be requested to make themselves available for promotion or publicity of SHIL and shall make themselves reasonably available for such purposes.
  17. The competition and the terms and conditions of this agreement will be governed by Scots Law and subject to the non-exclusive jurisdiction of the Scottish courts.
